CYBERWIZ: ASSESSMENT PREPARATION CHECKLIST

1. Name the decision
Write the question that prompted the assessment and who needs the answer.

2. Identify the systems
List the high-level services and systems the organization depends on. Do not include credentials.

3. Map responsibilities
Identify the internal owner and the external provider for each important system.

4. Collect existing requests
Gather relevant partner, insurer, funder, or leadership questions.

5. Find existing documentation
Locate current policies, prior assessments, and incident plans. Note what is missing.

6. Identify important access
Describe the types of staff and vendor access that need discussion.

7. Record constraints
Note staff availability, internal approval steps, and known dependencies.

8. Choose the participants
Identify who can authorize the scope and who can answer technical questions.

Do not include passwords or sensitive client records.
